Artificial Intelligence (AI) is reshaping sexual and reproductive health and rights (SRHR), offering unprecedented opportunities to expand access and personalise care. Yet as digital technologies proliferate, from chatbots answering sensitive questions to predictive models improving maternal outcomes, critical questions emerge about equity, bias, and who truly benefits from these innovations. Drawing insights from recent presentations on digital health transformation, this article examines both the promise and perils of AI in SRHR, highlighting the urgent need for inclusive, ethical approaches to technological development.
The World Health Organization's recent technical brief confirms that AI is already making significant inroads in SRHR, with applications spanning screening, prediction, and information provision through conversational agents. Current AI applications in SRHR focus on screening and predicting health concerns in areas such as infertility and pregnancy, as well as providing information through conversational agents or chatbots (World Health Organization, 2024).
AI-driven technologies are addressing longstanding barriers to reproductive healthcare. Fertility tracking applications and wearable devices help individuals monitor their reproductive health, while assisted reproductive technology increasingly relies on AI for embryo selection and sperm quality assessment. These developments are particularly valuable for underserved populations who may have limited access to traditional healthcare services.
Recent presentations at digital health conferences have highlighted innovative approaches to AI-powered healthcare support. Clare&me, a Berlin-based startup founded in 2021, demonstrates the potential for AI to provide anonymous, accessible mental health support through voice-based conversations (EU-Startups, 2024). While not directly focused on SRHR, such platforms illustrate how conversational AI can lower barriers to sensitive healthcare discussions and reach individuals who might otherwise lack support.
The platform addresses high entry barriers and waiting times by providing digital psychotherapeutic support via voice-based smartphone conversations, covering topics such as fear, stress and panic (EU-Startups, 2024). This model demonstrates the broader potential for empathetic AI tools to complement traditional SRHR services, particularly where stigma or accessibility present significant barriers.

Despite promising developments, integrating AI into SRHR raises serious ethical concerns that were prominently discussed in recent digital health presentations. AI systems are fundamentally limited by their training data, and biased or incomplete datasets can perpetuate discrimination, particularly against women, LGBTQ+ individuals, and people in low-resource settings.
Privacy represents another critical challenge. The sensitive nature of reproductive and sexual health data demands robust protection measures, yet current regulatory frameworks often lag behind technological capabilities. The digital divide further complicates the landscape, as unequal access to digital infrastructure and literacy risks leaving the most vulnerable populations behind.
Social media platforms, powered by AI algorithms, present a double-edged sword for SRHR. While these platforms enable community engagement, peer support, and advocacy, they also facilitate the rapid spread of misinformation about contraception, abortion, and sexual health. This misinformation can undermine public trust, distort health behaviours, and exacerbate stigma, highlighting the urgent need for responsible content moderation and improved digital literacy.
Recent presentations have emphasised how algorithmic amplification can inadvertently promote harmful content, making the development of ethical AI governance frameworks increasingly critical for public health.
The digital transformation of SRHR is not experienced equally across populations. Women, gender minorities, and marginalised groups often face greater barriers to accessing and benefiting from AI-driven health solutions. Research consistently shows that diverse stakeholder involvement in AI tool design and deployment is essential to ensure relevance, safety, and trustworthiness for all users.
The importance of representation extends beyond end-users to development teams and datasets themselves. Without intentional efforts to include diverse perspectives and experiences, AI tools risk serving only the digitally privileged while potentially harming those they purport to help.
Recent discussions in the digital health community have converged on several key principles for responsible AI implementation in SRHR:
Equity in Design: Meaningful involvement of end users, particularly those from marginalised communities, throughout the development process ensures tools are relevant, safe, and trusted.
Ethical Guardrails: Robust regulatory frameworks for data privacy, transparency, and accountability are essential, as outlined in recent WHO guidance on AI governance.
Inclusive Education: Investment in digital and AI literacy, particularly for women and underrepresented groups, empowers informed participation and oversight.
Cross-sector Collaboration: Partnerships between technologists, SRHR professionals, policymakers, and civil society are crucial for creating solutions that are both innovative and just.
The intersection of AI and SRHR presents both unprecedented opportunities and significant risks. Recent presentations and developments in the field demonstrate that the question is not whether to embrace these technologies, but how to ensure they serve as allies in advancing reproductive justice.
The decisions made today about AI development and deployment will determine whether these technologies become tools of empowerment or instruments of further marginalisation. By centring the voices and needs of those most affected by healthcare inequities, we can build a future where digital health innovations truly serve everyone, leaving no one behind.
